Initial Comments:
An off-site, state licensure follow-up survey conducted on May 26 and May 30, 2023 found that Revolutionary Hospice had corrected the deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a state re-licensure and state licensure complaint investigation survey completed on March 31, 2023,
